Records for the second opinion

What the second-opinion office asked for, what we sent, and what they have confirmed. Uploaded means it reached their system; attached means the coordinator confirms it is in Mom’s chart; read means a clinician has reviewed it. As of the September 8 call, we have confirmation of upload and attachment as listed below. Clinician review has not been confirmed.

  1. Addendum attached? Coordinator to call by Thursday, September 10. If nothing by Friday, September 11: son calls the second-opinion line. Fallback: Mom brings a printed copy on the 15th.
  2. Slides: the center is handling the request. Ask at the visit whether they arrived.
  3. Imaging disc: ask the coordinator when she calls back.
