Second-opinion intake call
Record type: my notes of a phone call, written during and right after the call. Who: me (daughter) with Mom on speaker, and the new-patient coordinator at the academic cancer center’s breast oncology second-opinion line. When: September 4, about 9:40 am. Call lasted about fifteen minutes.
What we asked for: a second opinion with breast medical oncology on the overall treatment plan for Mom’s newly diagnosed right breast cancer. Mom gave verbal consent for me to handle records and scheduling. The coordinator noted Mom’s consent and said Mom would also sign a release at the visit.
What they offered: an appointment on Tuesday, September 15, at 10:30 am with a breast medical oncologist. Arrive thirty minutes early for registration. A family member is welcome.
What they need before the visit (coordinator’s list, in the order she gave it):
- Imaging reports (mammogram, ultrasound).
- Pathology report(s) from the biopsy, including any addendum with receptor results.
- Pathology slides — they request these directly from the hospital pathology department; we do not need to arrange it. They will send the request today.
- Visit notes or after-visit summaries from breast surgery and medical oncology.
- A current medication list, which Mom can bring on the day.
How to send: upload through the center’s records upload page (link sent by email to me, with Mom’s permission), or fax. Uploads go to medical records and are attached to the chart “usually within two business days.” Their new-patient coordinator confirms receipt by email.
One question for the callback: whether they want the imaging itself (the actual pictures on a disc) or only the reports. Added to the question log.
Next step: upload items 1, 2, and 4 tomorrow. Mom will put the medication list in her bag. See the transfer record.
