Call to the second-opinion office about the addendum
Record type: my notes of a phone call. Who: me (daughter) and the new-patient coordinator at the academic cancer center. When: September 8, 2:10 pm. About six minutes.
Why I called: no reply yet to my September 7 email asking whether the pathology addendum had been attached to Mom’s chart.
What the coordinator said (my notes, as close to her words as I could get):
- She can see my upload from September 7 in the medical records queue.
- It has not yet been attached to the chart. Health Information Management works the queue in order; two business days is typical, so “probably tomorrow or Thursday.”
- She will check again on Thursday, September 10, and call or email me once it is attached. If I have not heard by Friday, I should call the line again.
- The slides request to the outside hospital is still open; nothing new.
What this means for us:
- The upload is confirmed received. The addendum is not yet attached to the chart, per today’s call. No clinician review has been confirmed.
- The next move belongs to the coordinator (a callback by Thursday). If there is no callback by Friday, September 11, my brother will call, since he is taking Mom to the visit. Which number to use
- If it is still not attached by September 15, Mom brings a printed copy of the addendum to the appointment.
