Receipt confirmation from the second-opinion office

Record type: email from the new-patient coordinator, and my same-day reply with one more upload. When: email received September 7, 11:32 am. My reply and upload at 7:05 pm.

Email from the coordinator (copied from my inbox):

Good morning. I have reviewed the chart ahead of the September 15 visit. The following outside records are attached and available to the physician:

  • Diagnostic mammogram and ultrasound report, 08/19/2026
  • Surgical pathology report, core needle biopsy, 08/26/2026
  • After-visit summaries, 08/28/2026 and 09/03/2026

Requested from the outside hospital, not yet received: pathology slides and blocks. This request was sent 09/04/2026; slides typically take 5 to 10 business days.

If a pathology addendum with receptor results (ER, PR, HER2) exists, please send it as soon as possible, as the physician will want it for the consultation.

Please have the patient bring a photo ID, insurance card, and a current medication list.

My reply, 7:05 pm (sent from my email, with Mom’s permission):

Thank you. The addendum with the receptor results was signed 09/02 and appeared in the portal yesterday. I have just uploaded it through the records link: surgical-pathology-report-SP26-XXXXX-addendum-1.pdf. Could you confirm when it is attached to the chart?

Automated upload confirmation, 7:06 pm: same wording as the September 5 confirmation: “We received 1 file(s)… This message confirms the upload only.”

Status as of this message: imaging report, biopsy report, and both visit summaries are confirmed attached to Mom’s chart. The addendum is uploaded but not confirmed attached. The center is handling the slide request with the hospital; receipt is still unconfirmed. Transfer record · The addendum
