SURGICAL PATHOLOGY REPORT
Collected: 08/22/2026 · Received: 08/22/2026 · Reported: 08/26/2026
CLINICAL HISTORY: 64-year-old woman. Right breast 10 o’clock, 4 cm from nipple, 1.8 cm irregular spiculated mass, BI-RADS 5. Ultrasound-guided core needle biopsy with clip placement.
SPECIMEN: A. Right breast, 10 o’clock, ultrasound-guided core needle biopsy.
FINAL DIAGNOSIS:
A. RIGHT BREAST, 10 O’CLOCK, CORE NEEDLE BIOPSY:
- INVASIVE BREAST CARCINOMA OF NO SPECIAL TYPE (DUCTAL), NOTTINGHAM HISTOLOGIC GRADE 2 (tubule formation 3, nuclear pleomorphism 2, mitotic count 1; total score 6 of 9).
- Ductal carcinoma in situ, intermediate nuclear grade, solid and cribriform patterns, present in association with the invasive carcinoma.
- Lymphovascular invasion: not identified in the sampled cores.
- Biopsy site changes present.
COMMENT: Prognostic and predictive marker studies (estrogen receptor, progesterone receptor, HER2, and Ki-67) have been ordered on block A1 and will be reported in a separate addendum. Definitive tumor size, margin status, and lymph node status cannot be determined on a core biopsy and await surgical excision if performed.
GROSS DESCRIPTION: Received in formalin labeled with the patient’s name and “right breast 10 o’clock” are four tan-white to tan-pink cylindrical tissue cores measuring 0.8 to 1.5 cm in length and 0.1 cm in diameter. Submitted entirely in one cassette (A1).
MICROSCOPIC DESCRIPTION: Sections show cores of fibrous breast tissue infiltrated by nests, cords, and occasional tubules of malignant epithelial cells with moderately pleomorphic nuclei and moderate amounts of eosinophilic cytoplasm, eliciting a desmoplastic stromal reaction. Mitotic figures are infrequent. Adjacent ducts are expanded by a proliferation of intermediate-grade malignant cells in solid and cribriform patterns without necrosis, consistent with ductal carcinoma in situ. No lymphovascular invasion is identified in the material examined.
